Improvements to our Long-Term Condition Recall System

You may recently have received an invitation from Parkway Medical Group for a long-term condition review.

We have recently improved the way we identify and recall patients who have long-term health conditions.

As part of this improvement, we are now proactively identifying more patients who are due or may benefit from routine monitoring and an annual review. This means that some patients may receive an invitation when they have not previously been contacted in this way.

Why have we changed this?

Our aim is to make sure that patients with long-term conditions receive appropriate monitoring and regular reviews, helping us to:

  • keep your condition under review;
  • check that your treatment remains appropriate;
  • carry out relevant blood tests and other monitoring;
  • identify any changes in your health at an early stage; and
  • make sure you receive the appropriate care and support.

Receiving an invitation does not necessarily mean that there is anything wrong or that there has been a change in your health. In many cases, it simply means that our improved recall system has identified that you are due for routine monitoring or review.

We appreciate that some patients may be surprised to receive an invitation if they have not received one previously. We are making this change to help ensure that our recall system is more consistent and that patients receive the appropriate care for their long-term conditions.
