Easy Read Privacy Notice
As from 1st April 2015, as part of our GMS contract, we are required to allocate a named, accountable GP to all of our patients, including children. All patients at this practice are allocated a named, accountable GP. When you are next at reception or contact us by telephone please ask who your named GP is, the receptionist will be able to give you this information. If you wish to change the name of your named, accountable GP to another GP we will do our best to accommodate your request. Please be aware that this does not restrict you to that named GP, you are free to see whichever GP you wish.
Extract of Fit Note Information
The Department of Health has advised us that at the end of January they will begin extracting information on Fit Notes (formerly called Sick Notes) from patient medical records. This information will be anonymised so individuals will not be identified. The data is being collected on behalf of the Department for Work and Pensions to:
- Undertake research analysis to inform policy relating to employment and sickness absence, including evaluation of the Fit for Work programme
- Help identify geographic level differences in return to work so that national and local providers can respond to provide appropriate services.
The surgery has no power to prevent this data extraction which will be retrospective to December 2014 and will continue until August 2018. Individuals may, however, opt out by completing the opt out form available under the Care Data section on this website or at reception.
